LIPOFILLING

Volume loss due to heredity, aging or disease can be solved by choosing a lipofilling treatment. Excess fat is removed by liposuction and then injected again at the place where you want more volume. Lipofilling is mainly applied to the breasts and buttocks, but it is also an effective and safe way to add volume to your face. Because your body’s own material is used, rejection or encapsulation after the lipofilling treatment does not occur. Depending on the size of the procedure, it is performed under general or local anesthesia. The effect of lipofilling is immediately visible after the procedure.

It is important for the surgeon to check where the fat is to be removed and the quality of your skin. It is important that you have a good and realistic picture of the final result.

LIPOFILLING

Heredity, aging or disease can cause local volume loss in particular in the face. A solution to this volume loss is lipofilling. Lipofilling is a technique whereby true liposuction fat cells are harvested and after processing are injected elsewhere into the body. There are many applications for lipofilling such as for the face, breasts, buttocks, and dents in the body. Besides fat cells there are also stem cells that are aspirated with liposuction.

Due to these stem cells the effect of lipofilling induces a softening of tissue like in scars, trauma, and burns or after radiation. With lipofilling the harvested fat is injected through small puncture holes that leave scars that are hardly visible. Because it is working with the body’s own fat, rejection or encapsulation after lipofilling does not occur.

DIFFERENT LIPOFILLING TREATMENTS

Aging, disposition and illness are causes for loss of volume, wrinkles and other irregularities of the face. Injecting synthetic filler is often effective in adding volume to the face where it is needed. Permanent fillers run the risk of inducing a foreign body reaction leading to palpable and visible nodules. Temporary fillers are safe, but, have the disadvantage of being temporary.

Lipofilling is an effective, durable and safe way to add volume to the face, and because lipofilling involves filling with your own fat, the chances of complications are limited. In the first couple of months the volume injected will decrease.

About 50 to 80 percent of the injected fat will survive. The natural aging process will continue and you may want to repeat a lipofilling treatment in the future.

Lipofilling of the breasts can be performed for different reasons. One breast may be smaller whist both breasts are similar in shape. Lipofilling may have several advantages for women who have had a lumpectomy and radiation of the breast.

Volume can be added, defects can be filled and the symmetry will be improved. In addition, the firmness of the breast due to radiation therapy can soften after lipofilling. The amount of fat that can be injected in one sitting is limited and not all fat cells will survive.

The percentage take may vary from 50 to 80 %. When a significant enlargement of the breasts is desired, it is often necessary to perform several lipofilling procedures. Alternatively, a more predictable and clearly visible enlargement can be obtained in one operation with breast prostheses.

Decrease of elasticity of the skin, loss of weight and pregnancy are possible causes of sagging and under filling of buttocks. A buttock enlargement through lipofilling may correct this. It is not only about enlarging the buttocks, but also about improving the shape. Areas with excess fat are reduced and fat is injected in areas that are deficient. Lipofilling of buttocks often involve large volumes of fat to be injected but there must be sufficient fat elsewhere in the body that can be removed without causing adverse effects.

The first consultation

During the first consultation it is determined if lipofilling will be able to solve your concerns and if the procedure will be able to meet your expectations. The procedure, what is to be expected after the procedure, the possible complications and the expected results will be discussed. It will be determined whet the fat should be harvested from and what the quality of your skin is like.

Lipofilling procedures follow a fixed sequence of actions. Usually the procedure is performed under general anesthesia, but small lipofilling procedures are performed under local anesthesia. After large volume lipofilling an overnight admission may be necessary, but otherwise the treatment takes place as a day case. In advance it will be determined in consultation with you where the fat will be harvested from. Most often this will be from the abdomen, flanks or the thigh area. The areas will be marked in advance, both the donor site as well as the recipient site. During the procedure an anesthetic solution with adrenaline will be injected into the donor site. Thin suction tubes are used to aspirate fat at a low pressure.

Then the harvested fat will be centrifuged to separate the living fat cells from the water and oil from the dead fat cells. These living fat cells are transferred to smaller syringes and injected through small cannulas in the predetermined areas. For these cells to survive it is of great importance that they are provided with oxygen and nutrients. The supply of this comes from nearby blood vessels.

Not all transferred fat cells will retain sufficient oxygen and some of them will die and be broken down and drained by the body. It is difficult to determine what percentage of fat cells will survive and depends on the area to be treated, the technique and the amount of fat to be injected. The duration of the procedure depends on the volume of fat to be injected.

After the procedure both the donor and the recipient site of the lipofilling will be somewhat swollen and bruised. In particular, after lipofilling of the face and around the eyes there will be significant swelling in the first week after the procedure. In the first 24 hours some watery fluid and sometimes blood may leak from the injection sites. Do not be concerned; this will settle on its own.

The treated areas are sensitive and the donor site feels tense. Sensitive bruising, swollen skin and a fair feeling are to be expected. In some areas fluid will accumulate, but this will all settle down in two to six weeks. You can experience some muscle pain as if you performed heavy exercise. The skin may also temporarily feel numb. You will experience more discomfort from the donor areas than from the recipient areas.

Pretty soon after the procedure you will be able to go back to work, but strenuous activities should be avoided for four to six weeks.

The effect of lipofilling is immediately visible, although part of it is swelling which will eventually disappear completely. Also, part of the volume will disappear because not all the fat cells will survive and will be cleared by the body.

In advance, it can be determined that a second or more lipofilling procedures are necessary to obtain the desired result. Sometimes a second procedure is necessary to correct irregularities or asymmetries. Only after three months can it be estimated how successful the procedure has been. You can determine the final result after one year when the healing process is complete.

When the injected fat has taken its final shape, it will not disappear anymore. The aging process however continues and in due course you may request another lipofilling treatment. Fat is always harvested from areas with excess fat and the improvement of shape thereafter is an added advantage of lipofilling.

Overall, lipofilling is a safe procedure. Nevertheless, there can be side effects. At the recipient site bleeding and swelling can occur which can result in asymmetries in the early operative period. Asymmetries and irregularities can be permanent when the fat cells do not survive equally in the different areas. Correction of significant asymmetries and irregularities is always possible.

In spite of preoperative antibiotics infections can occur, although very rare. Sometimes the fat is visible under thin skin or palpable as small lumps. Long term swelling can exist for months after the procedure. Sometimes oil cysts develop that can be aspirated. When large amounts of fat are injected, such as in breasts or buttocks, calcifications can develop. Micro calcifications are visible on mammograms and can mimic breast cancer.

It may therefore be necessary in these cases that additional investigations are performed.
